TERMS & CONDITIONS

1. Membership Structure
Membership is a monthly, one treatment per month, auto-renewing concierge program providing aesthetic services and privileges.
• Non-transferable
• No cash value
• Not insurance and not a substitute for medical care

2. Eligibility & Enrollment
• Must be 18 years or older
• Valid payment method required
• Medical consultation and consent required prior to treatment

3. Recurring Billing Authorization
By enrolling, Member authorizes The Few Institute for Aesthetic Plastic Surgery (“TFI”) to charge the payment method on file on a recurring monthly basis for membership fees, applicable taxes, and any additional services or products authorized by Member.
This authorization remains in effect until properly canceled in accordance with this Agreement.
Failure to maintain a valid payment method constitutes a material breach of this Agreement.

4. Billing & Payment Terms
• Membership fees are billed monthly on the enrollment date
• Failed payments must be resolved within five (5) days
• A $35 administrative fee may be applied to failed payments
• Membership benefits are suspended immediately upon payment failure

5. Minimum Commitment
An initial six (6) month commitment is required. Membership converts to month-to-month thereafter.

6. Early Termination / Recapture of Discounts
If Member cancels prior to completion of the minimum commitment period, Member agrees to pay the difference between:
(a) the retail value of all services received; and
(b) the total amount paid under membership pricing
This amount will be charged to the payment method on file upon cancellation.

7. Benefit Structure
• One (1) treatment per billing cycle
• Benefits do not accumulate beyond one billing cycle
• Maximum rollover: one (1) unused treatment, valid for 30 days only
• All unused benefits expire automatically
Unused benefits have no cash value and are not redeemable.

8. Appointment Policy
• Forty-eight (48) hours notice required for cancellation or rescheduling
• Late cancellations or no-shows result in forfeiture of that month’s treatment
• Same-day cancellation fees may apply
Repeated violations may result in termination without refund.

9. Upgrades & Downgrades
• Upgrades are effective immediately
• Downgrades permitted only after minimum commitment period with 30 days written notice

10. Add-On Services
Add-on treatments and services must be paid in full at the time of service.

11. Product Discounts
• Apply to regularly priced products only
• Cannot be combined with other promotions

12. Membership Freeze
Membership freezes are granted solely at the discretion of TFI and must be requested in writing.
• Supporting documentation may be required
• Maximum freeze period: three (3) months within a 12-month period
Freeze periods do not extend or alter the minimum commitment unless approved in writing.

13. Cancellation Policy
• Thirty (30) days written notice required
• Membership remains active through the billing cycle
• All unused benefits are forfeited upon cancellation

14. No Refund Policy
To the fullest extent permitted under Illinois law:
• Membership fees are non-refundable
• Services rendered are non-refundable
TFI may, at its sole discretion, provide prorated adjustment in cases of documented medical contraindication.

15. Medical Disclaimer
• All treatments are subject to provider evaluation and approval
• Results are not guaranteed
• Treatment plans may be modified at any time

16. Medical Discretion & Substitution
TFI reserves the right to modify, delay, or substitute treatments based on clinical judgment, safety considerations, or medical appropriateness.

17. Contraindications
Treatments may be postponed or denied due to:
• Pregnancy or breastfeeding
• Active skin conditions or infections
• Medications or recent procedures
• Any safety concern identified by provider

18. Member Responsibilities
Member agrees to:
• Provide accurate medical information
• Follow all pre- and post-treatment instructions
• Maintain respectful and appropriate conduct

19. Membership Misuse
Membership may be terminated without refund for:
• Sharing or transferring benefits
• Fraudulent activity
• Abusive or inappropriate behavior
• Repeated policy violations

20. Chargebacks & Payment Disputes
Member agrees not to initiate a chargeback without first providing written notice and allowing ten (10) business days for resolution.
Improper chargebacks constitute a material breach and Member agrees to reimburse TFI for all associated costs, including fees and collection expenses, to the extent permitted by law.

21. Limitation of Liability
To the fullest extent permitted by Illinois law:
TFI’s total liability shall not exceed the amount paid by Member in the preceding three (3) months.
TFI shall not be liable for indirect, incidental, or consequential damages.

22. Right to Refuse Service
TFI reserves the right to refuse service or terminate membership at any time for:
• Safety concerns
• Medical inappropriateness
• Disruptive or inappropriate behavior

23. Modifications to Program
TFI reserves the right to modify services, pricing, or benefits with thirty (30) days written notice.

24. Governing Law
This Agreement shall be governed by the laws of the State of Illinois.
Venue shall be exclusively in Cook County, Illinois.

25. Severability
If any provision of this Agreement is deemed invalid or unenforceable, all remaining provisions shall remain in full force and effect.
